This is an office visit to the surgeon.  A patient comes into the office after an abrasion to his forearm.  It will not stop bleeding. The surgeon cauterized to control the bleeding and sent him home with some cream.  How would I code this visit.  Would it be coded with an E/M level with modifier 25 or is there a specific code for cauterization to control bleeding? I would so appreciate help with this.  Thanks!</div>

He wrote, in part, &quot;The aortic valve was exposed via a hockey=stick type of incision. Initial findings were as described. It was impossible to access the prostehtic valve given the narrowing of the sinotubular junction by the rigid calcium shelf formed by teh existing bovine pericaridal patch. The ostium of the right coronary artery was identified and protected and then the aoric root between the right coronary artery and the midportion of the noncoronary cusp was completely excised.  This opened up the access to the bioprosthesis, which was explaned.  Annular fibrotic tissue and pledgeted matrial were completely debrided. A 23 mm St. Jude Regent was placed in a supra-annular position with interrupted 2-0 Ethibond stitches, reinforced with Teflon felt pledgets placed on the ventricular side of the annulus.  The aortic root was reconstructed with a new, large piece of bovine pericardium, significantly enlarging the area of the sinotubular junction.&quot;  <br />
I cannot find a code for the reconstruction.  It seems to me like this more than described in 33412 and definately not what is described 33864 since there was no &quot;valve sparing&quot;.  <br />
Can anyone suggest how this should be coded?<br />

			<content:encoded><![CDATA[<div>When CPT designates a code as Add-on with the '+' sign, the code is exempt from the multiple procedure concept. But the add-on codes are specific to one or more base codes.<br />
<br />
CPT says that the add-on codes are always performed in addition to the primary service procedure.<br />
<br />
What happens when the base code to which the add-on code is attached is not the primary surgical code but the base code is itself a secondary code subject to multiple procedure reduction?<br />
<br />
As an example:<br />
22802 - posterior arthodesis is primary<br />
22214 lumbar osteotomy is secondary subject to multiple procedure reduction<br />
BUT 22216 (billed for second and third level osteotomies) is an add-on code and NOT subject to multiple procedure reduction.<br />
<br />
Logically, one should reduce the 22216 because the base code to which it is attached 22214 has already been reduced - but it does not work that way.<br />
<br />
Thoughts would be greatly appreciated.</div>
