E/M with mod 25 and 11303

I billed blue cross the 99214-25 and then 11303. Should i have applied a 59 on the 11303? Or should i have billed 99214 then 11303-59. Thanks

I think you billed it the right way. -59 is an "unbundling" modifier and E&M codes aren't "bundled" with procedure codes, so the -25 is correct. Google some info about the -59 modifier and it may help to explain the circumstances of the use of -59 a litlle more clearly (probably better than I could, anyway...lol). Good luck!
