hammertoe with other p/cx

Is this the correct coding for this scenerio?

Partial Nail avulsion w maxtrixectomy - 11750

Exostectomy Hallux Medial - 11760

Arthroplasties interphalangeal joint of 4th & 5th toes - is this a hammertoe p/c since pt. did have hammertoes? i used 28285 w correcto modifiers.

Hemiphalangectomy 4th & 5th toe - 28160

so confused..please help )
