Encounter Form with Codes for Opthalmology

Looking for an encounter form for Ophthalmology - specifically Pediatrics. Need a list of codes to compile a proforma.

Google it, the internet is a wonderful sorce for examples, etc... I re-developed ours and used examples I found on the internet to help update and restructure our old version.
