Hello,

Our physicians keep stating History of COPD, History of Hypertension, History of Chronic Kidney Failure.

I was taught that if it is a condition that will never be cured or resolved you would code as current as chronic condition.

I am coding for professionial side not the faculity side.

Can anyone clarify on this?

Yes, I see the same thing....as a medical student/resident, providers are taught that language (patient has pmh of DM, HTN, etc when they have active conditions).

Ironically, on the page preceding the Z codes, there are general guidelines and under the heading "Apply History (of) Codes for Eradicated Conditions" it states "Personal History codes typically apply to conditions the patient used to have that have the potential to occur again".

My understanding (and what I teach my providers) is that if a condition is totally resolved, document in Past Medical History.
