Wiki 80307 & G0483 Frequency Denial

We are billing 80307 and G0483 CPTs and we are receiving rejection for frequency, insurance (Medicare NC, Medicaid NC and UHC) is not stating appropriate frequency range or time frame. If anyone came across this type of issue, please let us know if anybody is having any solution.
