One of our providers asked a question in ICD-10 training class yesterday that we are unable to answer. The specific scenario was a patient treated for cardiac arrest due to heart failure. Here's the coding challenge:

I46. - Cardiac arrest
I46.2 Cardiac arrest due to underlying cardiac condition,
with instruction to 'code first underlying cardiac condition"

I50. - Heart failure
This diagnosis category has Excludes 1 note for cardiac arrest (I46)

So.....how is this handled? Appreciate your expertise and help. :)

Sherryle:
I can say that during my training and information that I have obtained, you would code the I46.2 (cardiac arrest due to underlying cardiac condition) and code first for the heart failure. Example:

Patient has Chronic diastolic congestive heart failure and suffers cardiac arrest:

I50.32: Chronic diastolic (congestive) heart failure
I46.2: Cardiac arrest due to underlying cardiac condition.

Hope this helps :)
