Hello all,

Extracapsular cataract extraction with anterior vitrectomy

DX:
Hypermature cataract 366.18
Phacolytic glaucoma 365.51
Zonular Dehiscence ??

"Trypan blue was injected into the anterior capsule for better visualization. Hypermature cataract was moved into the anterior chamber. The phaco handpiece and the apatula were used to remove the lens nucleus. The remaining lens material and capsular bag were removed using the anterior vitrector. The capsular bag had to be removed from the eye due to its instability and inability to hold an IOL. The ant. vitrector was then used to do a thorough anterior vitrectomy until were no vitreous strands in the anterior chamber."

IOL was not implanted.

66852? and not sure about Zonular ICD-9?

I don't think 66852 is the correct procedure code. That's for a pars plana removal of lens, not anterior.

I would look at 66840 or 66850 and the anterior vitrectomy code 67010.
The only problem with using the 67010 with the cataract code is that anterior vitrectomy is not separately payable UNLESS you had documented in your notes in advance that you had planned to do it.
