Patient comes in with asthma attack - doctor bills e/m with meds; aerosol treatments; pulse ox - multiples - how would this be coded and with what modifiers - and do you see reimbursements on all these services?
 
this is a hard question answer with out the documentation, you could have prolonged time for the physician in addition to the E&M, If you have multiple treatments then you use the 76 modifier on those and a 25 on the visit level. most of the insurances will bundle the drugs into the nebs and the plus ox will bundle with most payers as well. With specific documentation my answer will be better.

Sightly improved aeration; now with hi pitched wheezes b/l and crackles over right posterior lung field. Impression & Recommendations:

Problem # 1: ASTHMA NOS W/ACUTE EXACERBATION (ICD-493.92)
Improved aeration and decreased wob as noted above after 3rd xopenex tx. Will increase xopenex mdi to 4-6 puffs q 4-6 hrs and start Flovent 2 puffs bid until acut sx's resolve then cont qd thereafter. Orpared and zithromax as prescribed.
Call for worsening or persistent sx's; sooner for increased wob or for fevers.

2) Zithromax 200 Mg/5ml Susr (Azithromycin) .... 6 ml by mouth once day one and then 3ml by mouth once daily for the next four days.
