A physician was forced to join a hospital group because he could not make a profit on his own. The hospital group audited his initial claims and found that his under coding would result in an over $120,000 a year loss. Upon discovery, the physician asked his previous staff and was that told they were apprehensive about questioning his coding.

An obstetrician failed to document an abnormal anal exam but referred the patient to a General Surgeon for a colonoscopy. The Coder wanted to ask the physician for a diagnosis to link to the colonoscopy referral because it was not time for a screening, but he had been up all night with deliveries and was non-approachable. By the time the General Surgeon called with the same question, the obstetrician said the referral must have been a mistake because the exam was marked as normal. Subsequently, the patient was diagnosed with colon cancer after a colonoscopy was ordered by the Primary physician two months later.

The coder, seeking additional documentation for an MRI, was hesitant to approach a physician a second time after her first attempt was met with irritation caused by a back up in the schedule. The MRI was subsequently denied. Patient left the practice saying "nice Doctor, but the office can’t do anything right. I’ll find an office that can."

A gastroenterologist was selecting codes on the superbill for diagnostic and screening colonoscopies. An audit of 25 claims showed that he was overpaid on 3 and underpaid on 15. The missing revenue affected his bonus structure and he did not receive the money he rightfully earned. The coder did not see the documentation due to delays in transcription time. She explained that she had been trying to get a meeting with him to provide coding education but he was always too busy.
