Default Documentation of Lab Orders

We are a pain management clinic that performs drug screens in the office with a tabletop reader. Our question is what type of documentation of physician orders are necessary for Mediare. Currently, the physician orders are in his office notes. Do we need a separate order sheet in the chart?
