New patient, detailed HPI, Exam was deffered, Low MDM. No time was documented.
All 3 components are not meant, would I ask the physician for a time spent with pt.

Why was exam deferred? Do you have vital signs and mood and affect at least? If so then it is a 99201

I can't answer why exam was deffered, our OB puts that down a lot on his patients.

Is it just the pelvic that was deferred? What all is documented?
