59 with add-on codes

I know that it is not appropriate to use modifier 51 with add-on codes. Is the same true with modifier 59? I have gotten a 2nd denial for 95886 as "not a valid code". It was billed with 95904 and no modifier appended.

This code is new for 2012 so you may want to check and make sure the insurance company has updated their fee schedule.

I add modifier 59 to nerve conduction codes only. Leave 95886 w/o modifier. I received payment fine so far from payers.
