By ICD 9 CM, that would be codes 414.01, 443.9, and 250.00.
However you would like to check for the highest specificity of these disease processes: as CAD can be of the native artery, extremity, etc; DM with manifestations, uncontrolled, Type 1 or Type 2.

Thanks. I was wondering if perhaps diabetes with manifestations codes should be used since the term "with" was used.
