Default Medicare Part B Reimbursement/Coding Changes

Hello,
I have been coding for Arkansas Medicare Part B for over a year now for the facility side. We always had to use the SG(Outpatient Surgery) Modifier that CMS Requires however recently the claims has been denying for C04 modifiers due to Novitas Inc took over. Do anyone know if I need to stop using the SG modifiers?
