Default amputation revision

29651 - is the neurectomy an automatic part of a further amputation or is it something that would have to be specifically stated in the op note?

Amputation, finger or thumb, primary or secondary, any joint or phalanx, single, including neurectomies; with direct closure

The physician removes a finger or thumb at any joint or phalanx, either right after an injury or infection or after an earlier attempt has failed. This procedure includes removal of vessels and nerves and a direct closure of the wound.

if there is a traumatic amputation of the finger, and the surgeon states he debrided the sharp end of the bone and removed the nail and nail matrix and closed the wound with a v-y advancement flap, is this a revision amputation 26952 or just a debridement with a flap closure (14040, 11044)?
