Default orders

In outpatient hospital coding, if a patient has hypertension and takes home meds for it but DID NOT receive the meds during the stay in the hospital would it be appropriate to code hypertension?
If a lab test was ordered and there is no diagnosis stated as the reason for the test, would it be appropriate to code hypertension if it covers the test?
