Default ICD-9 Procedure Codes

I code for an Ambulatory Surgery Center and have never encountered this coding issue before. I called Fireman's Fund to follow up on a Workers' Compensation claim that was billed on a UB 04. They said that my claim was rejected because Box 74 requires an ICD-9 PROCEDURE code. Where can I locate a crosswalk listing between CPT codes and ICD-9 Procedure codes?

I know Volume 3 of the ICD-9 book is the tabular list for procedures but I do not know of a crosswalk to make it easier to find.
