Please add me to your distribution list. And can you answer a question for me? I am doing an audit but all E&M documentation is just scribbles by the provider. I can't decifer anything! Yet he billed mostly level 4s!!!! What do I do? Are hand-written notes even acceptable documentation or are they suppose to formally dictate? Please email me. A good topic would be 'What is the difference in chart, medical records and coding audits". I have been ask this question several times and it seems you can perform each of these seperate and not really audit the other. Depending on how fragmented the practice departments are I know audits can be done seperate but my opinion is, making sure the chart and medical records are correct and not auditing the code billed is wasting time. Audting should be making sure your codes are correctly billed according to componentes necessary to bill the apporiate level of service. Am I correct?. Then audit the chart for correct demographics. Your coding audit should cover the documentation and chart audit. Then compliance for all componentes of the chart are meet.
