Question History of chemo-induced neutropenia

I am trying to find the most accurate code for "history of chemo-induced neutropenia" to bill for our patients who are no longer actively neutropenic. Any advice?

Is that all they are being seen for, with no active problems?? They are just following up? I would just use V-codes for the cancer and the chemo.

Question

Well the pt. still has active cancer and is currently undergoing treatment. But I wanted to also code the "history of neutropenia" as a supporting dx on the office visit.
