Default Billing an E&M during global days

Hey,
I have a scenario I need help with: Pt comes in 5/1/10 for knee and nose lac. Knee is sutured and nose is closed with dermabond(tissue adhesive). Pt returns apprx 7 days later for suture removal from the knee. However, the nasal lac is still oozing and dermabond is re-applied.
I feel that we can charge a lac charge again for the dermabond with a 78 (I think) modifier (return to OR). The surgery guidelines state that if there is complications, exacerbations or RECURRENCE then it doesn't fall under the global pkg. Anyone have an opinion?

What if the wound got infected and antibiotics had to be prescribed? Could you bill an E&M during the post-op period?
