smoking cessation and preventative medicine visit in conjunction with E&M

Has anyone billed the code 99406 for smoking cessation and gotten paid, what did the doctor put in the chart for documentation?

Also, for visits E&M in conjunction with 99401-99404 has anyone billed these and gotten paid for. What type of documentation was used to substantiate both visits? Acatlett
