Default Ophthalmology-glasses lens re-check

If a patient had their eye exam on 3/4/10 and returned on 6/9/10 because they wanted their glasses Rx adjusted b/c it was hard for them to see with, is there a certain code that is used for patients who are strictly coming in b/c their lens strength isn't working correctly for them? Or is it just the E/M codes? it was also mentioned in the brief note that pt. felt like there was a foreign body in her eye but nothing was found...
