Default Neuropsychologist billing

I would like to know what the "normal" is on this for 96116 and 96116. Do you see the docs trying to bill the medical reason for the eval (such as memory loss or brain injury) or are most docs billing the after affect such as cognitive DO as the primary diagnosis?
