If the provider did not document the time spent - would you say it does not support a billable service or would you recommend a subsequent hospital visit or outpatient visit depending on the setting?

Also what documentation would you expect to see for 90817?

If it is evident it is a therapy note, I would ask provider to addend and add it. I dont support doing addendums often, but if he truly forgot it, than I would havehim add it. Otherwise, yes, go to a 9923_ code.
