Default minor surgery notes

I would like to know if anyone can give me the basics of minor procedure notes. Here is my scenario, the office where I work just hired a Gyn Onc. He performs various biopsies in the office, such as colpos, vaginal or vulva bx., etc. I know what I think a note should have, location, size, etc. I would just like to have others opinions so I will be prepared when I speak with the physcian.
