What CPT code would you use to report the use of Altepase medication instilled into the port to unblock a port that is clotted?

36593..declotting by thromboltic agent of implanted vascular device or catheter...

Would a facility charge 36593 if the procedure was performed by an RN?

Yes the facility would charge it. We are a hospital based outpatient and use 36593 as long as its on an order.
