Default Office Based Anesthesia

Does anyone have experience with coding office based
anesthesia? I have few drugs that not sure how to bill:

Naloxone 0.4mg/ml - Found code for J2310 however it is for 1mg.
Is this drug up coded or use J3490?

Midazolam PF 1mg/ml and 5mg/ml. Are both coded as J2250 and
units based on amount used?

Default Need assistance with office based anestheais

The provider is billing E1399 for oxygen billed by liter, back up power source (portable power source) and portable oxygen tank cart. The provider says they take all this to the office with them in addition to other supplies and drugs...Are the E1399 supplies even payable?
