Default Modifier 33

I want to make sure I'm using Modifier 33 correctly. I work for a pediatric practice & am putting Modifier 33 on preventative procedures done in addition to a well visit/exam. For example if we see a pt for a 4month check up I am applying the modifier to ALL immunizations & admin codes for that visit. Is that correct? I'm also using it on eye exams (99173), hemoglobins (85018) & developmental testing (96110).

And also, am I to put a modifier 25 on the well exam?

Just want to make sure I'm reading/understanding the code correctly. I've found that currently in Missouri Anthem & Healthlink are requiring the use of this modifier, yet their staff is not yet well educated on the use themselves to advise us adequately.
