Default Suture of a J-Tube

what if a patient comes in with there J-Tube loose or suture came out, and the physician sutures it, how would this be billed ? they don't give cm and it's not a wound, but it is repaired just like a wound !!!

would it be included in the fee for the placement of the J-Tube ? what if it was placed at another facility ??
