Default Infant exposed to Paxil during pregnancy

Physician documented that mother was concerned about exposing her infant child to Paxil while she was pregnant. The child has no effects noted from this exposure. Physician assessment states: "History of Paxil exposure". Should we note this diagnosis and, if so, what code would we use? Thanks for any help anyone can provide!
