Default Hosp OP Discontinued Proc

A planned LHC was initiated but the phys was unable to gain access in the left radial, anesthesia was administered and equipment setup for the case. Unsure of the correct cpt codes to use with modifier -74. Would I bill cpt 93458 w/-74 ???

You will bill the procedure that was planned with the 74 modifier. Also you need a dx code as a secondary diagnosis from the V64.x category
