Default TAH and appendectomy at same time

Doctor attempted a LAVH. Had to convert to open due to adhesions. After freeing adhesion off the pelvic sidewall and the omentum the note reads:

The appendix had been adherent to the uterus posteriorly that had been dissected off. It was irritated and inflamed at this point." So the appendix was removed.

I feel that this would be cpt 44955 but what diagnosis? Please correct me on CPT if wrong also.
