Physician Fluoride Treatment

Hi,

The codes for Physician Fluoride Varnish Services have been deleted in 2011. e.g. D0145 (oral evaluation) and D1201 through D1206 (topical application of fluoride).

what should we use to replace them?

Thanks in advance for any help and advice.

DO145 AND D1246 to be billed together per Medicaid guidelines
