Default Reasonable and Customary

Is there a reference book out there for reasonable and customary by geographic location? I've been googling but can't find one.

If I am correct in assuming you are referring to "usual and customary", I do not think there is one due to so many variables it would be exhaustive.
Usual and customary is based on providers of the same or similar practice, in the same geographical location. My humble opinion (interpretation) of this is they have surveyed the given area and taken the average/mean cost and used that as their base.
