Default Asthma and Allergy

I have a provider that bills 95165. I know that we can only submit 20 units per DOS with Medicare and up until recenly had been able to bill 120 units per DOS with BCBS. My allergist physician recently billed on claim for 60 units and another for 120 units. The claim for 60 units paid but the 120 units denied. Does anyone know what the maximum amount of units that can be billed per DOS with BCBS?

I am not sure what the limits are because the maximum that we bill to Anthem is the 60 units. Are you really billing 8 vials of serum at one time? I know that our 60 unit charge is for 4 vials and lasts the patient from 9 months to a year.
