Default Hyperhydrosis billing?

We provide the service "Hyperhydrosis" (medical application of botox). Is anyone else billing for this service and getting it paid? If so, how? We are not able to get this service paid by carriers at this time.
Any comments appreciated!

Can you supply some more details. How are you billing this out? What is the reason for the denial? Substantial criteria needs to be documented if I remember correctly in order for these procedures to be paid...
