Question PFSH Question

If a physician documents two PFSH during a hosptial H&P or a consult would this be considered complete, or would a complete require all three PFSH be documentated?

For a new patient, all three PFSH needs to be documented for both 1995 and 1997 guidelines. This means Past, Family and Social history all must be documented.

- Consultations
- Initial Hospital Care
- Initial Observation Care
- Initial NF
-


Require all three, Past, Family and Social history to be documented for a comprehensive history
