We saw a patient and the doctor dictated as the diagnosis "clearance for anticoaulation" - any suggestions on what I should use?

Hi. There has to be a medical reason why the patient is being cleared for anticoaglation therapy. If there isn't anything else documented in the patients record other than anticoag clearance, I would question the physician....
