Default Laser treatment for nail fungus

Is it acceptable to bill insurance for laser treatment of nail fungus ? 17110 is what the Dr wants to bill. LCD for this cpt does not accept 110.1 there is 686.8 unspecified tissue infection that is approved for this CPT and Dr wants to bill as this. This does not seem appropriate, can anyone advise?
