Default Developing Documentation Program for Residents?

I was wondering if anyone has put together a documentation program for incoming residents or if you know where I can find info about getting one started?

I am working on one--although mine is aimed more at the Attending providers who supervise the Residents.

If we can reach an agreement about its use, I may be able to provide it as a sample. PM me or email me at kevbshields@yahoo.com

Keep in mind that it may be awhile before I could consider it "complete."
