ICD-10 Monitor’s Talk Ten Tuesday interviews this week included Lauri Gray, RHIT, CPC, discussing the changes in the 2013 ICD-10 draft code set.
“We’ve got changes to some of the coding conventions and the general coding guidelines, as well as some chapter-specific changes,” said Ms. Gray. “One of the bigger changes relates to a general coding guideline… If a provider documents a borderline diagnosis at the time of discharge, the diagnosis is coded as confirmed unless the classification provides a specific entry, such as that for borderline diabetes. If a borderline condition has a specific index entry then that code is used.”
