CPT® 2010: Major Changes for CCT

Cardiac computed tomography (CCT) professionals will see major changes in coding next year. CPT® 2010 adds four new Category I codes to report CCT and cardiac computed tomography angiography (CCTA) services and deletes four Category III codes.

In a statement posted on its Web site prior to the Oct. 30 release of the 2010 Outpatient Prospective Payment System (OPPS) final rule, the Society of Cardiovascular Computed Tomography (SCCT) said it did not foresee significant changes in payment for CCT/CCTA. They were singing another tune Nov. 2.

Prior to the 2010 OPPS final rule, SCCT stated that, for hospitals, it anticipated that “CMS will assign the new CCT/CCTA Category I codes in similar APC groupings [as 2009 AMA Category III ‘T’ codes] when the final HOPPS rule is released in November 2009.”

After reviewing the final rule, however, SCCT said, “There are several notable components to this rule which are of particular concern to SCCT members.”

Of particular concern to the SCCT are the 2010 reimbursement rates for CCT/CCTA codes. “A preliminary review indicates there may be errors in valuation for some of the practice expense calculations for the codes,” the SCCT states.

Here are the new CPT® 2010 Category I codes for CCT/CCTA:

CPT® Code Description
75571 Computed tomography, heart, without contrast material, with quantitative evaluation of coronary calcium
75572 Computed tomography, heart, with contrast material, for evaluation of cardiac structure and morphology (including 3D image postprocessing, assessment of cardiac function, and evaluation of venous structures, if performed)
75573 Computed tomography, heart, with contrast material, for evaluation of cardiac structure and morphology in the setting of congenital heart disease (including 3D image postprocessing, assessment of LV cardiac function, RV structure and function and evaluation of venous structures, if performed)
75574 Computed tomographic angiography, heart, coronary arteries and bypass grafts (when present), with contrast material, including 3D image postprocessing (including evaluation of cardiac structure and morphology, assessment of cardiac function, and evaluation of venous structures, if performed)

According to the SCCT, the 2010 interim payment rates for these codes are:

  • 75571 $94.59
  • 72272 $107.94
  • 75573 $144.87
  • 75574 $456.77

CPT® codes 75571-75574 replace Category III codes 0144T, 0145T, 0150T, and 0146T-0149T, respectively. The American Medical Association (AMA) instructs physicians not to bill 75571 with 75572-75574. According to SCCT, potential crosswalk codes exist for codes 75572-75574.

    If the CT heart (cardiac structure, morphology and 3D) is performed with calcium scoring.We need to bill only 75572
