Correction to Home PT/INR Monitoring Service

The Centers for Medicare & Medicaid Services (CMS) issued, Jan. 8, Change Request (CR) 6313 to correct an omission of ICD-9-CM diagnosis codes reportable for prothrombin time and international normalized ratio (PT/INR) anticoagulation management monitoring services furnished on or after March 19, 2008.

CR6313 replaces CR6138, issued July 25, 2008, and adds the following ICD-9-CM codes for PT/INR anticoagulation management monitoring services: 451.11, 451.19, 451.2, 451.80-451.84, 451.89, 453.40-453.49. and 415.12.

Last year, CMS revised its National Coverage Determination (NCD) on home PT/INR anticoagulation management monitoring services furnished on or after March 19, 2008 to expand coverage to patients on warfarin with: 1) mechanical heart valves; 2) chronic atrial fibrillation; and 3) venous thromboembolism (inclusive of deep venous thrombosis and pulmonary embolism).

    # 4 – adding 5th digit to 596 seems to be an error as there are no 5th digits in the listing.
