CPT 23700: Shoulder Manipulation With Anesthesia

When reporting CPT® 23700 Manipulation under anesthesia, shoulder joint, including application of fixation apparatus (dislocation excluded) general anesthesia—not local, moderate sedation, etc., is required. Per CPT Assistant (April 2005):

CPT code 23700 is intended to be reported for the manipulation only when performed under general anesthesia. The code descriptors, which include the phrase “requiring anesthesia” or “under anesthesia,” indicate that the work involved in that specific procedure requires the use of general anesthesia; therefore, it would not be appropriate to report code 23700 if general anesthesia is not provided.

    I have a client who had a fixation of shoulder and she had (manipulation under anesthesia-Oolo-Austin Trigenics manual capsular dissection) procedure code 23700. Her insurance company denied the claim as ” experimental/Investigational or not Medically Necessary”. The client said that other people had their claims paid. The claim was appealed and it was still denied. Do you have any idea how this procedure and be more insurance friendly and if there is another code that would be better to use. This has cost my client over $7800 out of pocket.

    adhesive rt shoulder capsulitis and pxs were major injection to the shoulder jt (20610) and right shoulder manipulation, is the manipulation bundled to the major injection?
