Medicare creates physician specialty codes to identify the type of medicine a physician practices, such as cardiology, internal medicine, or general surgery. These codes describe the specific and unique types of medicine that physicians practice and are used by the Centers for Medicare & Medicaid Services (CMS) for claims processing and programmatic purposes, such as policy administration, data analysis, reporting, and oversight.
May 27, physician specialty code F7 was issued to identify a physician who self-designates as a Physician Nutrition Specialist® (PNS) when enrolling or updating provider enrollment through CMS-855I, CMS-855O, or the Provider Enrollment, Chain, and Ownership System (PECOS). It applies to dates of service beginning Oct.1. Medicare systems will implement F7 on Oct. 5, according to CMS Manual System Transmittal 13765, New Physician Specialty Code for Physician Nutrition Specialist (F7).
Physician Nutrition Specialist Qualifications
A PNS is a physician with advanced nutrition training who dedicates a large portion of their career to nutrition. A PNS may lead and coordinate interdisciplinary clinical nutrition services and education in academic health and medical centers, private practices, and other health care settings. These physicians typically have backgrounds in specialties, such as:
- Internal medicine
- Pediatrics
- Family medicine
- General surgery
A PNS may also have particular expertise in subspecialties such as adult or pediatric gastroenterology, endocrinology, critical care, nephrology, cardiology, etc. They must complete defined nutrition training that includes core knowledge and residency training, which includes a period of mentored clinical nutrition experience. PNS must also meet all requirements for certification by the National Board of Physician Nutrition Specialists.
What You Need to Know Before Implementation on Oct. 5.
How F7 implementation affects medical practices and coders:
- It is not a CPT®, HCPCS Level II, or ICD-10-CM code or modifier, so medical coders generally do not select F7 on the claim. The physician or enrollment staff selects F7 in the enrollment record.
- The specialty code is tied to the rendering physician’s Medicare enrollment record and transmitted for claims-processing purposes.
- Confirm that physicians practicing nutrition medicine have updated their Medicare enrollment information appropriately.
- Expect claim-editing, utilization-review, payment, and reporting systems to distinguish physician nutrition specialist from other specialties.
- If a claim is denied because of a specialty-related edit, verify that the physician’s enrollment specialty is correct — but do not assume that F7 alone establishes coverage or medical necessity.
- Continue assigning diagnosis and procedure codes from provider documentation under the usual coding and payer rules.
The change gives CMS a distinct way to identify physician nutrition specialists. It does not create a new billable service, change documentation requirements by itself, or guarantee payment for nutrition-related services.
