Hello everyone!

Does anybody know the correct way to terminate a patient from a practice? Is there a form? should we let patients insurance know the reason of why we are terminating patient from our practice? Any guidance is really appreciated. Thank you in advance

If the patient has a government insurance plan, I recommend that you contact the carrier to find out what steps they require. If the patient is not on a government plan, you can refer to information on the OIG website.

There are legal requirements when you discharge a patient from the practice. You do not typically need to notify the insurance unless you are the selected PCP for a plan that requires selecting a PCP.
Some (but not all) key points:
1) You should terminate in writing, via certified mail.
2) You must continue to be available to the patient for a specified amount of time to allow them to find another provider (typically 30 days from notice).
3) Include a medical release form with the notice.

Remember that a patient you need to discharge is typically an already upset patient just looking for another reason to complain or pursue litigation. You want to appear as logical, compassionate & reasonable in this situation.
