I have a case when provider used forceps and scissors to excised vulvar warts (7-9) and he sent them to pathology. There is no other documentation in reference to margin sizes. Should I use this set of codes or the 11420- to code this?
 
Destruction is not equal to excision. Destruction codes would be used for application of acid, use of laser, cautery, etc. Additionally, the 1142x codes require a full-thickness excision, ie, they cut down into the dermis. If they are just snipping/shaving at the surface, it is not a full thickness excision.
 
Thank you, I will query md just to make sure this was not just a shaving using scissors. And in case that provider just shaved the lesion what should be the code?

The codes for shaving a lesion of the genitalia are 11305-11308 but these require a size. The default would be the smallest size. The tangential Bx codes might also apply and are by lesion, not lesion size 11102-11103.
