    Wiki 2020 location?

I wish the AAPC would record the sessions and offer them as a virtual conference. I am on a drug for MS that does a number on my immune system so I avoid large crowds such as this, especially since the dates always fall a couple of weeks after the infusion. Not to mention I usually still feel...

    1997 Exam Bullets

I audit our GYN and Neuro docs using the 1997 specialty exams. I need an opinion....I have researched this and cannot find an answer. If a "bullet" cannot be performed in the exam due to circumstances beyond provider control, should they be penalized and not allowed an otherwise comprehensive...

    Data points for MDM

I would be careful giving a point for reviewing a report and 2 points for reviewing the images for the same study. Our MAC stated in a webinar that is is an either/or situation, not both.

    MDM for rectal prolapse

Does the patient have chronic conditions or a co-morbidity that would make the surgery high-risk? That is something the physician would have to determine and document.

    CEMC Study / Audit Tools

I laminated my audit tool and used a dry erase marker for the both the CEMC and CPMA exam. I found that to be easier than dealing with 25 or more individual audit forms. Good luck!

    HCC Trump List

Good Morning! I am in need of the latest HCC "Trump" list. I have been searching but the one I did find was from 2014. If there is a newer one, could someone please point me in the right direction? Thank you!

    ROS question

Novitas does allow you to use information from the HPI as both HPI elements and ROS elements. I came across a letter from 1998 where the executive medical director for HCFA (now CMS) stated "it is not necessary to mention an item of history twice in order to meet the Documentation Guidelines...

This is a question for those who have taken and passed the CDEO exam. Would you please share the sources/materials you used to study for this certification? Medicare requesting office records

Check your local carrier's website. It should have the information regarding pre-pay audits. Our carrier, Novitas, has it listed under Medical Review, Upcoming Focus. The list is extensive including office procedures as well as multiple E/M codes. And watch your EOBs......you never get a...

CPMA CEUs: how do I get them?

Mike, I noticed that once I hit the required number of CMPA CEUs, they automatically go to the CPC. It's the same with my CEMC. I don't understand why, but at least we still have our CPMA covered!

I have a membership to NAMAS. They provide 12 CEUs yearly for the CMPA.

E/M Documentation for 2 or more problems, help!

I must disagree with the above statement regarding Novitas allowing credit for location twice in the HPI. They have said in recent E/M webinars on using their audit sheet that an HPI element can only be counted one time. You can, however, count something in the HPI and as a ROS.

    Help with Stroke ICD-10 Guidelines

What is the "time frame" for using the ICD-10 codes for cerebral infarction (I60-I63 series) versus cerebral infarction with residual effects (I69 series) or personal history of cerebral infarction with no residual effects (Z86.73)? I remember reading an article that basically stated you would...

I agree with the above post. I do understand it is hard to find a place that will make everyone happy, but Nashville would be nice.....it is driving distance for me. I'm sure an adequate location could be found in Dallas or St. Louis as well. I don't fly any more because of health issues so...

    99201 for nurse only visit

I think it will depend somewhat on your state's scope of practice for RN's and if the RN could be credentialed to be able to bill insurance. I would not bill those visits as incident-to under the supervising physician because the physician has not seen the patient to establish a plan of care...

data points for review of mri report

You cannot give credit for both independent visualization and review of the report for the same image; it is an either/or situation. The physician just needs to document HIS interpretation of the images, it doesn't have to be a "formal" report. Now if your doc orders additional radiology, then...

MDM review of old records and or obtain history other than patient.

I work with a pediatric group. We do not give credit for obtaining history from someone other than the patient when the patient is an infant/young child.....it is a "given" that the parent/guardian will be giving the history. One of our pediatricians even gave us an article a few years ago...

Coding chronic conditions with preventive well-checks

I respectively disagree with the above statement. The Part B News article quotes at least 3 industry experts who state the excludes note does not exclude billing a problem visit on the same day as a preventive visit. The title of the article is "ICD-10 Z-codes don't mean you can't bill...

    Medicare Review Audit

    proficiency exam

Does anyone know why the AAPC changed the length of time to pass the proficiency exam? Originally, when ICD-10 was to go into effect 10/1/14, we had one year before and one year after implementation to pass the test. Now, with the implementation date of 10/1/15, we only have until 12/31/15. I...

    1997 GYN Exam Question

Under Gastrointestinal, one bullet is "Obtain stool sample for occult blood when indicated". Obviously, an occult blood test is not...

    Medicare ADR Process Questions

Novitas will down-code the claim to what they feel is the correct code. I definitely would not recommend letting the ADR "time-out"; I feel that practice would put you on their radar more than if they down-code some claims. Also, if they do down-code a claim, be sure to audit it yourself; I...

    4x4 method

That means if the 4x4 (4 things documented in 4 organ systems or 4 body areas) isn't met, an auditor can still determine that an exam is detailed rather than expanded problem focused. Novitas also does not allow you to mix body areas and organ systems....it has to be one or the other.
