1. I am new at coding in a hospital, I had been in a specialty office. When the ED dx is sore throat or pharyngitis do I really use 462 - Acute pharyngitis? 784.1 - throat pain excludes sore throat but I feel like I am changing the dx by using the acute code.

2. I know that if I get a diabetic code for neuropathy like 357.2 I need to also have 250.6x, but if I get 250.6x without a manifestation, can the 250.6x stand alone? I mean this for any of the diabetic codes with manifestation.

I think you are correct going with 462. This is listed under sore throat in both the index and the tabular sections.

For diabetes w/manifestations I believe you always need to specifiy the manifestation after the 250.xx code for all complications related to DM.

I would need to see the statement by the ER physician for the pharyngitis code, for the diabetes the manifestation code is required when you use the fourth digit indicating manifestation.
